Who We Are
The Central Coast Community Health Workers/Promotores (CHW/P) Collaborative brings together healthcare providers, community-based organizations, county agencies, educational institutions, and managed care plans across Santa Barbara, San Luis Obispo, and Ventura Counties.
Together, we’re working to strengthen and sustain the Community Health Worker / Promotor(a) / Representative (CHW/P/R) workforce — the trusted individuals who bridge gaps in care, advance health equity, and serve their communities with cultural knowledge and compassion. Just testing
Our Mission
To resource and uplift the CHW/P workforce across the Central Coast by supporting long-term sustainability, strengthening partnerships, and promoting systems change that centers equity, collaboration, and cultural responsiveness.
What We Do
The Central Coast CHWP Collaborative strengthens the Community Health Worker/Promotor(a) (CHW/P) field through three core areas of action:
Our Collaborative Model
Our governance structure centers partner voice and lived experience. CHW/P supervisors and those with firsthand experience inform decisions through an inclusive, partner-driven process.
Administrative coordination is provided by the Santa Barbara Foundation, while each organization retains autonomy in implementation.
