What We Do

The Central Coast CHWP Collaborative strengthens the Community Health Worker/Promotor(a) (CHW/P) field through three core areas of action:

We support skill development and workforce sustainability by coordinating regional training opportunities, identifying shared learning needs, and connecting CHW/Ps with pathways for professional growth.

We facilitate collaboration among partners through meetings, shared resources, and data exchange that promote alignment across counties and organizations. By connecting local voices, we ensure that best practices and emerging insights are shared widely.

We elevate community experiences to inform systems change. Through collaborative advocacy, data storytelling, and policy engagement, we help ensure that local and regional decisions center cultural responsiveness, equity, and the lived experience of CHW/Ps.

Our Collaborative Model

Our governance structure centers partner voice and lived experience. CHW/P supervisors and those with firsthand experience inform decisions through an inclusive, partner-driven process.

Administrative coordination is provided by the Santa Barbara Foundation, while each organization retains autonomy in implementation.

California State University, Channel Islands (CSUCI)
Located in Camarillo, Ventura County, CSUCI is a public university offering diverse academic programs.

CenCal Health (Medi-Cal Managed Care Plan)
CenCal Health provides low-cost health insurance plans in California for individuals, students, and families.

Center for Family Strengthening: Promotores (San Luis Obispo County)
This organization builds, fosters, and supports a network of Promotores throughout San Luis Obispo County by providing training, emotional support, and materials to empower a healthier community.

Children and Family Resource Services – Cottage Health
Cottage Children’s Medical Center offers extensive health care services including acute care, surgery, PICU, NICU, orthopedics, trauma, and pediatric specialties.

Center for Population Health – Cottage Health
The Cottage Center for Population Health works to improve health and wellness, focusing on equity for the most vulnerable in hospitals and the community.

Corazon Latino

Corazón Latino
Corazón Latino is committed to addressing the urgent and unique mental healthcare needs of underrepresented populations along the Central Coast. They offer mental health support groups, outreach initiatives, educational capsule videos, and free virtual classes to promote familial well-being.

Dignity Health – St. John’s Regional Medical Center (Oxnard)
St. John’s Regional Medical Center is dedicated to delivering high-quality, compassionate care to Oxnard and nearby communities.

Dignity Health – Marian Regional Medical Center (Santa Maria)
Marian Regional Medical Center offers many services, including back and spine services, orthopedic care, and cardiac care.

Family Service Agency (FSA)
FSA provides mental health and family support services to individuals and families in Santa Barbara County.

First 5 Santa Barbara County
First 5 Santa Barbara County supports the health, early learning, and well-being of children prenatal through age 5 and their families.

First 5 Ventura County
First 5 Ventura County invests in programs and services for children from prenatal through age 5 to ensure they are healthy, nurtured, and prepared for kindergarten.

Gold Coast Health Plan (Medi-Cal Managed Care Plan)
Gold Coast Health Plan provides health care services to Medi-Cal beneficiaries in Ventura County.

Lompoc Valley Community Healthcare Organization
This organization focuses on improving the health and well-being of the Lompoc Valley community through collaborative partnerships.

Mixteco Indigena Community Organizing Project (MICOP)
MICOP supports, organizes, and empowers the indigenous immigrant community in Ventura and Santa Barbara Counties.

Planned Parenthood California Central Coast (PPCCC)
PPCCC provides comprehensive reproductive health care services in the California Central Coast region.

Ventura County Public Health
Ventura County Public Health works to protect and improve the health and well-being of the community through various programs and services.
